Medication Calculation and Prescription Writing
Instructions:
 Independently complete the five pediatric dosage calculations.
 Show dosage calculation for each medication.
 Provide a prescription for each medication – Write the prescription as if giving to a patient.
 Complete prescriptions using template located in Modules – Week 2
 Submit the dosage calculation and the written prescription for each medication by the due date
1. Child with Otitis Media (OM)
– Age: 7 years Weight: 66 lbs
– Medication: Amoxicillin Oral Suspension, 400 mg/5mL
– Pediatric dose: 80 mg/kg/day, every 12 hours, for 10 days.
2. Child with Croup
– Age: 4 years Weight: 33 lbs
– Medication: Prednisolone Oral Syrup, 25 mg/5mL
– Pediatric dose: 1 mg/kg/day, every 12 hours, for 3 days.
3. Infant with Fever
– Age: 9 months Weight: 20 lbs
– Medication: Acetaminophen Oral Suspension, 160 mg/5 mL
– Pediatric dose: 15 mg/kg every 4 hours for 3 days.
4. Adolescent Female with Pneumonia
– Age: 16 years Weight: 110 lbs
– Medication: Azithromycin Oral Suspension, 200 mg/5 mL
– Pediatric dose: 10 mg/kg on day 1, followed by 5 mg/kg daily days 2 to 5
5. Child with Tinea Capiti
– Age: 10 years Weight: 81 lbs
– Medication: Griseofulvin Microsize Oral Solution, 125 mg/5 mL
-Pediatric dose:10 mg/kg/day, every 12 hours, for 42 days.
